hello po,

ask ko lang po pano ang case pag nag change of name yung primary applicant due to marriage..will there be any conflicts
sa skills assessment or visa application since i change my surname, followed the surname of my husband? Magkaiba po kc ang surname s credentials ko nung single ako..now that i'm married,all my passports and other credentials is named under my new surname. Hope somebody in the group can enlighten me..thank you very much..

there is nothing to worry about this issue... you can prove that with your marriage certificate..
alam nmn nila na sa phils eh ginagamit ng female ang surname ng husband pag kasal na..

Para lang hindi ka magkaanxiety attack best way is to draft a letter and explaining why there is a chance of name. Include in your letter which among your documents apektado then have your letter checked and notarised...libre sa munisipyo or sa PAO or sa private law firm.

Sir ask ko lang po..now pa lang kc ako naka complete ng requirements. .tingin nyo po..need ko ng include yung marriage cert ko po s pagpasa ko ng application for skills assessment? Magkaiba po kc surname ko s TOR and passport ko now...

@bachuchay yes.. Include ctced marriage license to show you got married.. As proof of change of surname.. Goodluck..

meron ako ganyan. maya pag-uwi ko sa house isend ko syo yun letter (nakalimutan ko kc yun tawag sa letter) na provide ng phil. embassy saakin.(ofw kc ako)

supportiving documents ko na submit sa embassy:
1. old passport copy
2. new passport copy
3. marriage certificate.

submit ko sa phil embassy na documents.
1. copy of old passport po (and present ko din yun original passport) sknila.
2. copy of new passport with new name. (married name na). -present ko din yun original passport
3. yun copy marriage certificate . - present ko ctc na marriage certificate.

then may ibibigay syo ang embassy na letter Certifying na si single name and si Married name ay iisang tao lng. attached with your supporting documents. nakanotorized na din yun.

yun ang plan ko isubmit sa EA assessment . i Just want to make it sure na. alam nila isang tao lng yun nasa diploma ,TOR and yun mga working documents ko ngayon.

check ko yun mga documents last nyt.
certificate from the embassy yun letter na yun. that stating that si Mrs. SingleMarried and Miss Single, birthdate XX. holding passport XXXX. are the same person. then nakaattached yun mga documents mo. and nakanottary na din. signed by the Vice Consul.

yes po magkabatchmate tyo. eto nga naghahanap n ng mga documents.
we're (ni husby) planning next month april. hoping matapos nmin yun CDR.

Hi @spawn08, san kyo kumuha ng gantang letter stating na si singke name and married name us one abd the same oerson? San pong philippine embassy? Currently nasa pinas ako, san kya ako pwede kumuHa? Plan ko kasi magrenew ng passport.

Ako naman po maiden name ko pa yung passport ko and other docs. Mga after three weeks pa makukuha yung nso copy ng marriage cert namen. Kelangan po ba pachange ko na yung passport ko?then how bout other docs? Kelangan din po yung married name na yung nakalgay na name ko sa mga docs ko?

Also need advise on my case. I am the primary applicant kasi and my current passport still bears my maiden name. Passport is about to expire in January and we're also expecting a baby next month. So ang plan ko is to have both my baby's new passport and my name change (passport renewal) be processed together in June.

The thing is, I am awaiting for my EOI result so lahat ng documents ko are still under my maiden name. Same lang ba ung process na pagdadaanan ko as per your above advise if I get a positive EOI result and continue with the next step (Skill Select)? Is there a need to fillout the change in circumstances form (1022?)?

What if hindi muna ako mag name change during passport renewal (I have checked with hubby and he is amenable with this hehehhe), how will it affect my application naman since both baby and hubby are my dependents? Or would you recommend na mag name change na talaga ako?

Hope you can help me on this. Your advise will be greatly appreciated. :)

hello @kremitz. kahit maiden name pa gamit mo ok pa yun. its up to you naman kung magpapalit k. pero kung nakalodge n kyo ng EOI. yan ang d ko alam. ask @lock_code2004 . baka alam nya. wala pa kc ako sa stage ng EOI kaya d ko pa alam.

kung tatanoningin mo ako. kung ganyan ang nasa position ko. hindi na muna ako magpapalit. kc nakapag EOI ka na. baka kc maging ok na, then nakapagpalit ikaw baka mas mahirapan ka ayusin sa DIAC. madali lng naman magpalit ng passport basta complete lng yun documents mo. intayin mo nlng muna yun nlng muna yun result ng EOI mo. pag may visa ka na. for me sa Phil Embassy AU ka nlng magpalit.
ask mo din yun mga idols dto like @lock_code2004. kung ano option nila.

NO issues with your maiden name in your passport
- submit your assessment
- submit your EOI
- process your visa application

if ever you decide to change your passport to your married name
- before the assessment, no problem, your marriage cert is enough to show you are married
- before EOI, no problem as well...
- during the visa processing, no problem, just submit Form 1022: Notification of change

For additional dependents:
- for pregnant- proceed with your processing as usual, then pagnagka CO na, inform your CO
- for new baby, just submit the same form 1022
